Personal service setting notification

Under the Personal Service Settings Regulation (136/18) there is a requirement for any personal service setting to notify the Health Unit 14 days in advance of commencing operation, adding a new service to an existing facility or renovating their facility. You can complete this notification by completing the form. A public health inspector will be assigned to follow-up with you.
