Welcome to the Public Health Ordering System (PHOS) Online Registration. PHOS is an online platform for Healthcare Providers in Middlesex County and the City of London to access and order publicly funded vaccines and public health resources.  

This registration form should be completed by the individual at each organization who is the primary contact for vaccine ordering and inventory.  For offices who order resources only (no vaccines), the role of the primary contact is to complete this survey, be the first office member with access to PHOS, and add initial office users in PHOS.

The primary contact will be required to provide an email address that will be used to log in to PHOS. Once access to PHOS is granted, additional users can be requested under the "My Profile" page in PHOS.
